Passport Services for U.S. Citizens

Passport services are available to U.S. citizens by appointment only.  U.S. citizens can request to receive their new U.S. passport/Consular Report of Birth Abroad via courier service.  Please see requirements for mail services below. U.S. embassies and consulates no longer issue U.S. passports locally except in emergency cases. As such, applications are accepted at the Embassy and forwarded to the United States for completion.

This process normally takes a minimum of one week, so we encourage all U.S. citizens to submit their applications well in advance of any travel plans to avoid unnecessary inconveniences.

Passport pickup hours are Sunday – Thursday from 11:00 am to 12:00 pm.

Any person over 16 years of age or older is considered an adult for the purposes of applying for a U.S. passport. When you come in for your passport application, please bring the following to the Consular Section:

  1. Your Current or Expired U.S. Passport. If you are not in possession of your passport, or if it was lost or stolen, please see the section below on Lost/Stolen Passports.
  2. Passport Application Form DS-82, (86 KB) filled out electronically and printed.
  3. Two (2) Identical Photographs, in color, taken within the last six months. These photographs must measure 2×2 inches (5×5 cm) with a white background. The photographs should provide a clear, frontal, full-face view, and be taken in normal street attire without a hat or glasses. Please ensure the photos you bring meet all passport photo requirements to avoid delays.
  4. Passport Fee of $110 USD, payable in cash (U.S. Dollars or Omani Rial), or by credit card.
  5. Schedule your Appointment Online
  6. If you wish to have your passport couriered back to you once services are complete, please bring a pre-paid Fed-Ex envelope with a return address with you at the time of your appointment.  Envelopes cannot be purchased at the Embassy. It is advised that applicants write down the tracking number upon purchase of the envelope, as the Embassy does not maintain this information. You may use the tracking number to track the return delivery of your passport online.

Any child under 16 years of age is considered a minor for the purposes of applying for a U.S. passport. When you come in for a passport application with a minor, please bring the following documents with you to the Consular Section:

  1. The Minor’s Current or Expired U.S. Passport. If you are not in possession of the child’s passport, or if it was lost or stolen, please see the section below on Lost/Stolen Passports.
  2. Passport Application Form DS-11 filled out electronically and printed.
  3. Two (2) Identical Photographs, in color, taken within the last six months. These photographs must measure 2×2 inches (5×5 cm) with a white background.   4. Proof of the Minor’s Relationship to the Parents, such as:
    – Original U.S. birth certificate showing names of both parents.
    – Consular Report of Birth Abroad showing names of both parents.
  5. Passport Fee of $115 USD, payable in cash (U.S. Dollars or Omani Rial), or by credit card.
  6. Schedule your Appointment Online
  7. Both Parents Must Authorize the issuance of the child’s passport. The easiest way to do this is for both parents/guardians to accompany the child in person to the Embassy.
  8. If you wish to have your passport couriered back to you once services are complete, please bring a pre-paid Fed-Ex envelope with a return address with you at the time of your appointment.  Envelopes cannot be purchased at the Embassy. It is advised that applicants write down the tracking number upon purchase of the envelope, as the Embassy does not maintain this information. You may use the tracking number to track the return delivery of your passport online.

If both parents cannot be present, please read the following and choose the scenario that applies to your situation:

You have sole legal authority You must submit evidence of this with the application. Examples include:

  • Complete court order granting you sole legal custody of the child, such as a divorce decree or other custody order
  • Complete court order specifically permitting you to apply for your child’s passport (photocopy is acceptable)
  • Certified copy of the child’s birth certificate listing you as the only parent
  • Certified copy of an adoption decree listing you as the only parent
  • Certified copy of the judicial declaration of incompetence of the parent that cannot appear in person
  • Certified copy of the death certificate of the parent that cannot appear in person
One parent unable to appear If one parent/guardian cannot go with the child to apply for the passport, they can give permission by completing Form DS-3053 “Statement of Consent.” (PDF 52.0 KB) You must submit the completed form with the child’s passport application. The parent that cannot go with the child must:

  • Sign and date Form DS-3053 in the presence of a certified notary public, and
  • Submit a photocopy of the front and back side of the ID that they present to the notary public with Form DS-3053.
You cannot locate the other parent You must submit Form DS-5525 “Statement of Exigent/Special Family Circumstances.” (PDF 44.0 KB)

  • Please complete the form with as much detail as possible.
  • We may ask for additional information (e.g., custody order, incarceration order, restraining order) to protect against international parental child abduction.
Neither parent able to appear A third party may apply for the child’s passport with a notarized statement from both parents/guardians giving that third party permission to apply for the child.

  • The statement must include a photocopy of the parents/guardians’ identification.
  • When the statement is from only one parent/guardian, the third party must present evidence of sole custody of the consenting parent/guardian.

    – The Consular Officer may consider other forms of citizenship and identity that you provide, but can only issue a new passport if convinced of your identity and U.S. citizenship.

  • Correcting a data or printing error
  • Replacing a limited validity passport ( only if issued within less than a year)

If you would like to have your passport couriered back to you once services are complete, please include a pre-paid return Fed Ex envelope with a return address when submitting the additional documents. (NOTE: The U.S. Embassy in Muscat will not accept envelopes from other courier services at this time.)

Also Important to Note: You may have the envelope delivered to your home address or one of the FedEx offices for pick-up. Families need only purchase one envelope if they are applying together.  If you fail to bring a pre-paid return Fed Ex envelope with a return address when submitting the additional documents, you will need to come to the Embassy to pick up your passport.

Note: If you schedule an appointment for passport services, please remember to bring two forms of identification. The guards will need to keep one form of identification while you are in the building.
